It sounds fishy to me. Are you sure they really pay?
I'm completely sure. I've received $3,000. That means that I have over $3,000 in my possession because of SWATcash. I've used the money to shop for clothes, school, etc. without a problem.

What exactly do we have to do to get money?
SWATcash is what is described as an offers site. You complete offers to earn money. Some of the offers include buying services like ordering a Blockbuster subscription, and others are simply filling out surveys. You are never required to spend money in order to earn it.
To learn more about offers on SWATcash, I'll once again reference you to the Official SWATcash Field Manual. Section IX of the guide talks about what offers are, how to do them, and gives some other tips for completing them.
06/15 UPDATE: The B (Brandarama) offers are approving very well! I highly suggest doing them as soon as possible. This is an easy $5 if you do all ten.
You will not become rich overnight, but in the long-run you will benefit. There are other ways to earn besides doing offers, but many of those ways are in addition to doing offers, not in place of doing them. An example of this is completing the first set of missions; once you complete them, you will be able to open the safe, which can help you find bullets, grenades, SWATcash, and real money. The highest real sum found has been $50. That's pretty good for just clicking a button once a day.

Misc. Tips
- When you join SWATcash, use your regular email address. However, for offers, use a different email address if you don't want spam.
- Check out the offer "SWATsurvey#1" in the Earn tab. You will get $1 for doing it and all you have to do is answer how you found the site.
- Join the SWATcommunity forums. It's a great way to keep up to date on all of the features and contests of SWATcash.
